Pacific Family Medical Practice is an AGPAL Accredited General Practice, servicing the Calamvale and Sunnybank Hills area since 2005 .Our experienced Doctors and staff are committed to providing quality Family Medical care.

Appointments

Patients with appointments will have preference except in emergencies. You can make an appointment with our Doctors by calling our reception team on 07 3711 2280 or book online.

15 minutes is regarded as a standard consultation in which one problem is addressed. If you feel more time with the doctor is required, you are welcome to request a longer appointment. Inform our friendly reception at time of booking. Please book an appointment for each family member.

Cancellations

Please remember to cancel your appointment if no longer required, please let reception know as soon as possible so we are able to offer the time to someone else.

Cancellation or Non-attendance fee of $40 applies unless 24 hour notice is given.

Upon Arrival

Please present your Medicare card and all Concession cards to the receptionists. It is important that you let us know if any of your contact details have changed.

Specialist Referrals & Medical Certificates

These are legal documents and we are unable to backdate them. We require you to make an appointment for any specialist referral, medical certificate or prescription/s.

Results and Reports

An appointment with your Doctor is required to discuss all pathology results and reports . You will be contacted if results require urgent attention.

Recalls and Reminders

Your health is important to us, our practice has a system whereby patients are contacted for follow-up and preventative health care activities.

Privacy & Confidentiality

All Health Service Providers must comply with the Privacy Act 1988 (Commonwealth). The Act incorporates Australian privacy principles 2018 which set the rules for the handing of personal information.
These principles cover collection, use, storage and disclosure of information.
Fully informed an voluntary consent as to the above must be obtained before or as soon as practicable after the collection of health information.
Information regarding a person’s medical history is needed to assist in providing diagnosis and treatment. To ensure quality and continuity of care, a Patient’s health information may have to be shared with other health care providers.
